What happens if lichen sclerosus is left untreated?
Untreated lichen sclerosus can progress, causing severe itching, pain, scarring, narrowing of the vaginal opening, sexual difficulties, and—in rare cases—increased risk of vulval cancer.
Detailed Medical Explanation
Without treatment, lichen sclerosus typically worsens over time. Chronic inflammation leads to permanent changes such as scarring, fusion of the labia, narrowing of the vaginal entrance (stenosis), and distortion of genital anatomy. Painful sex, recurrent tears, and urinary issues may develop. The lifetime risk of vulval cancer rises from about 0.3% to 3–5% if uncontrolled. Early intervention is crucial.
